Social Worker (MSW)

Hours: Mon - fri w weekends
The duties of the Social Worker (MSW) may include, but are not limited to: Conducting a complete psychosocial assessment of the patient and family and participating in the development of the plan of care at the time of the patient admission. Delivering or supervising the delivery of social services to the patient or patient’s family. Reviewing and updating the plan of care as often as necessary and reporting off to various hospital departments as required. Obtaining physician’s orders for services as necessary. Assisting the patient and family with identifying and accessing community resources. Reporting any changes in the emotional or social condition of the patient or family to the attending physician. Acting as a consultant to all caregivers in the patient care delivery model. Preparing clinical notes in compliance with regulatory agencies and Wellmont Health System policy and procedure. Participating in discharge planning and assessing the capacity of home caretakers to cope with the patient’s needs in order to develop the plan of care as it relates to the patient’s care after discharge. .. Assessing special needs related to cultural diversity including communication, space, role of family members and special traditions for care delivery. Identifying obstacles to compliance and assisting in understanding the goals of interventions for caregivers and patient. Identifying patient/family needs when discharge occurs or when level of care changes.
Minimum Masters Degree with major studies in social work, sociology, or psychology from a 4 year college or university accredited by the Council on Social Work Education. 2 years experience in case work or counseling in a health care or social services delivery system. Experience in an inpatient care setting preferred.
CPR is required for any RN, LPN, PCT, Patient Transporter or other direct patient care provider. Emergency Department, Intensive Care Units, PACU, Medical Emergency Team and Anesthesia require ACLS. Pediatrics, SICU, Emergency Department and Medical Emergency Team also require PALS
