Birth Certificate Clerk

The Birth Certificate Clerk is responsible for collecting the necessary data elements for the completion of the birth certificate as required by state law. Responsibilities also include working with the parents to gather data that is required for the Acknowledgment of Paternity (AOP). Duties Include but are not limited to: Interacts with the parents to collect and document the birth information, delivering the appropriate forms to them and providing guidance in the completion of the forms. Works with the parents to complete the Acknowledgment of Paternity form, which can require patience, diplomacy and sensitivity if there is conflict regarding parental responsibility. Reviews patient medical record in EPIC and other resources, as needed, to obtain required birth information. Reviews completed birth certificate worksheet for completeness and accuracy, ensuring that signatures of the parents are obtained for the birth certificate. Enters birth certificate information into appropriate computer software program (WINEBC) and transmits data in a timely manner as required by state law. Meets or exceeds productivity standard of processing two (2) birth registrations per hour. Changes baby names in EPIC as needed, from generic baby girl/baby boy to given name and validates name change three days post discharge. Maintains confidentiality with all PHI and HIPAA related information. Practices and adheres to the Wellmont Health System Code of Conduct philosophy and the Mission and Value statement. Files electronic fetal monitoring strips as required. Assembles, analyzes, records and enters birth certificate and paternity acknowledgement information within two days after delivery of infant (Must be submitted within 10 days of delivery).Handles post discharge phone calls from parents. Responsible for completion of Fetal Death Report. Other duties as assigned.
Two to three years in acute care setting in HIM department required. Prior experience as birth certificate clerk in hospital setting preferred. Demonstrates excellent interpersonal skills, ability to communicate orally and in writing, and attention to detail. Ability to handle and manage conflict.
BLS is required for any RN, LPN, PCT, Patient Transporter or other direct patient care provider. Emergency Department, Intensive Care Units, PACU, Medical Emergency Team and Anesthesia require ACLS. Pediatrics, SICU, Emergency Department and Medical Emergency Team also require PALS.
